When do you need to address/be concerned about gaps in employment?

I left my first job out of college 5 months ago after working there for 3+ years to relocate and change career paths after earning my MBA. I’ve been applying to jobs everyday (200+ applications).

I finally got a couple offers a month ago but turned them down because I got very sick and one was a 30% pay cut and the other seemed sketchy/unethical.

I’ve gotten asked a couple times now what I’ve been doing for the past five months and am wondering if I should just address it right away on my resume?

I left my most recent employer purposely during a slow time so I could train my replacement and work ahead a couple months. This allowed me to leave on excellent terms but the extra work on top of my final semester of my mba left no time to look for a job before leaving. It’s taken a lot longer than expected to find a new job and I’m starting to freak out.
