First sentence quitting a job in person

I am planning to resign from my job and can easily describe the reason why.

One thing I'm not sure on is how to open the meeting with my manager. What is the literal first thing you say to set things in motion? E.g. "Hi, thanks for meeting me today. I wish to give you my 4 weeks notice to resign from company", and then go into reasons.
