[seeking advice] Returning from medical leave, given less than stellar options, want opinions/feedback/advice

Hello everyone!This is a throwaway account.

So I live in a major metropolitan area in the US. I've been on medical leave and have been discussing options for returning to work. I was given the following options, all of which are less than desirable:

1. Relocate, same role, same pay: What this would entail is working in an area that is about 100 miles away from where I currently live.I would be open to this, except that I am currently in a lease, which would be extremely expensive to break (way more than I could afford). When I asked about relocation assistance, was told none could be offered.This would be about a 3 hour commute one way and be extremely expensive.
2. Severance package equivalent to 3 weeks pay.
3. Pay cut/demotion. This would be part time and not pay as much, I would lose health insurance from going part time to full time (I have medical issues and having the insurance greatly helps in keeping my costs way down, especially for expensive prescription drugs).
4. (provided only after I inquired) Apply to an internal position and be referred by current management. I don't quite feel qualified for any available internal positions at this time, one I feel I might be able to do with some extra training on a skill I don't have, but I don't have super high confidence on it. No guarantees on acquiring the role.

I am wondering what your thoughts are on these options. Do you think there's any room for negotiation on any of these offerings? Do you know anything about breaking out of a lease if I picked option 1? (buying out my lease isn't a financially feasible option). If I took the severance package, could I apply for unemployment? Should I hit the ground running looking for other positions with different companies? Is there anything else I should be asking? Help!!
