Rules re: finding coverage for employees who call out sick or request PTO in advance.

A coworker posed the following question to me and seeing that HR is out for the weekend, I wasn't sure what the correct answer is. For reference, I work in a hospital and this person works in a different department.

Question:

"If someone requests PTO and the manager approves it, who's responsibility is it to get that shift covered?"

Also:

"If an employee calls out last minute, who's responsibility is it to call other employees to get coverage or to get the next shift to come in early?"

My thoughts are the manager should be the one to call employees, seeing as overtime has to be approved by a supervisor/manager beforehand. The employee calling out should not be responsible for finding coverage if they're sick, and most definitely shouldn't have to find coverage if they use pre-approved PTO.
