Gap Years and Disability

I'm looking to get back into the workforce after spending the last three years or so on disability due to mental health issues. Right now it's a bit of a challenge searching for jobs because it feels like I've changed tremendously since before I went on disability. I was wondering if anyone with experience coming back from gap years and/or disability could provide some advice.

For the record, I have a B.S. in Psychology and I have five years of retail experience as a shift supervisor.
