What should I do?

I just accepted this job as an assistant. The manager specifically said I should be flexible. I told him I have evening classes but I can be flexible. He stated in the job contract, that I should be available from 9-6pm. I have therapy and physical therapy due to herniated disks with sciatica. The only time I can go to physical therapy is from 7:30-8:30am. They close at 5pm so I can go any other time. What should I do? I need physical therapy, especially since I’ll be seating in a desk chair for hours causing more pain.
