FMLA question

I asked HR a week ago to submit a blank FMLA form to my psychiatrist and she still hasn’t. How do I deal with this? I have really bad mental health and even thinking about asking her again makes me want to puke. It took me a month to work up the courage to finally do it.

I’m sorry if this is silly or not the right sub. I’m just at a loss. My work will literally do whatever they can to not help people. Pay people. Etc.
