Why doesn't it count as an absence/call out when employees clock out in the middle of the day (like for an appointment)?

I'm not saying it should count. I'd like to copy them. But I'm not sure if I should ask my supervisors since I'm not sure if it's actually allowed.

In my office I see people clocking out to take phone calls or leave for an appointment and come back in a few hours. Or some people clock out for an hour to take an unpaid break...

Is this something you'd usually be fired for, or is this okay? Why is it not seen as an occurrence?

In my office we are allowed to make up our hours by working later the next day, but these people don't seem to be doing that.

My job has a very high turnover rate. So I can't tell if these people simply don't care and are prepared to quit if my supervisors threaten them with termination. Or if this is a common thing that's okay to do.
