Jobless, just offput/scared/unsure/whatever, I don't know what to do. Looking for ANY work.

I am trying to put in a paper application at a local fast food place. The requests/requirements of past jobs have me so twisted and flustered, I can't fill this application out. I want to just waltz in tomorrow to talk to the hiring manager because I am intimidated by this stupid slip of paper. Everything they're asking is going to be answered stupidly. "Yes, they have worked here." period. So why are they so intent? Why am I so scared of it? I just don't know what the hell is wrong with me. I could do so much but I am scared of so little.
