UK Specific Job Search Support - Any Suggestions?

I need support to get back into work - I've been unemployed for over a decade due to disability (ASD & ADHD), my disabilities don't stop me working but they make looking for work close to impossible for multiple reasons - I need support in looking at whether I'd be better off in work, determining what jobs to look for, and addressing practical barriers like addressing my employment gap.

I've been to many job coaches but either 1. They just sit me in front of a PC to job search with no support, 2. They meet me once a month for coffee...and that's it, or 3. They refuse to work with me full-stop as they decide I mus be lazy/unwilling to work. Mainstream companies don't seem capable of supporting disabled clients and often they come with the risk of benefits being sanctioned if you don't do what they tell you (even if it's a detriment to your health and/or your job search), while disabled companies presume lack of competence - essentially, I'm both too disabled and not disabled enough.

I'm currently on ESA-SG so I've no contact with the job centre at all any more, even when I was going to the job centre they weren't able to suggest anything to help me, I've self-referred to most places.

I'm beyond frustrated, I desperately want to work but need support - so checking here on the off chance anyone knows of anywhere that may help/that I've not already tried, based in NorthEast.
