Do I have to be diagnosed to mark "yes" on a Voluntary Self-Identification of Disability Form?

It's in the title. I think I suffer from depression and/or anxiety, but have never been formally evaluated. I just want to know if I'd be breaking a rule by answering "yes"

And while I'm here, the name I'm supposed to put on the form is my legal name, correct? Full legal name? or just first and last?
