HR lady is being difficult about PTO... Is there a reason?

I work for a small company and in the last 3 years I've been here our paychecks while coming out to the correct amount, our vacation and sick time hours are always screwed. They either display a negative amount such as -72 hours PTO / 12 hours Sick or just are all messed up. In the past, this has led to me missing out on days that I deserved to have!

Therefore I took it upon myself to document my sick and vacation time, scan all the paperwork for myself prior to submitting it. As of today, our hours are still messed up in our paychecks. When I address this with her our (HR/Accountant) lady in charge of it, she states that its not her fault and it's the vendors we have used 2 vendors now but it's the same excuse, and she has a lot on her plate as is (she mentions). I mean it's really bad excuses. She says we can ask her anytime we want to know she has a spreadsheet of it. I think it's a poor excuse, but I don't understand what is difficult about this. I spoke to our payroll vendor and they said it's automatic, it's not difficult to do.

Is there a reason that she is giving me a hard time, something that perhaps benefits the company?

Lastly, I'm a month away from reaching my maximum hours. I have decided to take a vacation in September so I can use those days. But she says she only submits my hours to the pay period that closely relates to that month. I asked is there a reason, she said No it's just easy to keep count of that way. So if I'm taking a vacation in Sept but I'm close to reaching my max vacation pool now I can't withdraw my hours I have earned now for Sept. Therefore I have to take a few days off this month or next month just for the sake of keeping my Vacation hours down.

Lastly, she thinks I'm already over my vacation max, while she is wrong and I have proof this time when I review this with her later this week. I asked why she didn't tell me I was going over, and she got defensive.

I'm not going to let anger take over, this is frustrating. What can I do as an employee in CA to protect myself and politely put her in her place? She is generally a very nice lady but gets defensive when we talk about paycheck amounts or vacation/sick. I don't know why, but I think I deserve at the very least to know what my hours are when I receive my check instead of some random numbers that change every paycheck.
