Need surgery but just started new job

I have severe carpel tunnel in one of my wrists that requires surgery but I just started at a new job less than a month ago. The symptoms are getting annoying to deal with. I'll need at least a few days off though a week is preferable. I don't start accruing PTO yet and taking days off without it add points towards termination.

Any loop holes around this or do i wait q few months until I get enough PTO?
