Leaving job I hate

Almost a year now, I have been working a job I have wanted to quit since May. In june I started actively looking for a new job. After multiple interviews and offers, I finally got an offer that I like. Two things. First, how do I write a resignation letter for a job and boss I hate? Second, my former manager gave me such a great reference for this new position. I want to thank him for the reference. Is this usually done just by calling them and thanking them?
