How to respond when feel misrepresented in a performance evaluation?

My indirect manager provided an evaluation about me that I disagree with and I ended up with a really low score. I don't know what to do now and just feel so bad.

They wrote about things they had not seen in person and I think they don't really know what happened to begin with. I felt what was said was very biased and they had added an element of imagination to what really happened. But of course, those reading the evaluation just believe the manger and don't know that the managers have never seen what they were talking about.

For example, the evaluation said I did not work well with team members when actually the other team members had said they couldn't help me because they had urgent work to do. The manager also said I have executive functioning issues without giving any explanation. They had given me many tasks which I felt were leading nowhere (no matter what I tried, it won't give the result that we want), but I tried my best to keep doing what they said. The evaluation also included a cherrypicked incident when I had not done well. Much of what was said were very vague about what happened so it's hard to criticise directly what was said.

I haven't been feeling great. I feel maybe I am as bad as they describe, but part of me feels what they described is misrepresenting what happened. Does anyone have any advice for me? Have you been in this situation? I want to explain myself but I don't know where to start, whether they will listen and am worried it might hurt me instead.
