Leaving a job after 3 months - does it go on my resume?

Hey all - so, leaving a job after 3 months. In short, it’s just not the right fit for me. I have found that I want to work in a different field and came into this position because of insecurity in my previous job (high likelihood of lay-offs / salary cuts). Happy to go into detail if necessary, but onto my question!

As I now send out resumes in search of a new job, (while of course still giving my current job my best effort) should I list my current position? It is an accounting job and I’m looking to move into an HR assistant job (previous job had bigger HR focus). Do I leave this job out and show a gap on my resume? Do I explain why I’m leaving a job so early in my cover letter?
