Extend a job offer on the spot? Is it a good idea?

Is it a good idea to extend a job offer on the spot if you are waiting to hear back from another job? The first one is fast food and I am scared that extending it would make me look bad and they would just replace me with another candidate. However, I really want to hear back from the other job and work there if I am hired. Please help
