How to approach permanent remote request

Currently I am still with a job in this environment, but I have a big problem: I'm planning to move in the late summer to live with my partner. I was already planning this before the world blew up into a pandemic, but now I know I don't want to be in any dense city areas for a long, long time.

Currently I'm in New York and I'm going to be leaving to move to the Harrisburg, PA area. I don't really love my job and I would prefer to cut my losses and leave, but with the COVID-19 situation I'm very nervous about the prospect of quitting and entering a job market with few job openings and a lot of seekers.

Right now I am working remote and while it isn't perfect I don't find my job any harder as a result of being remote. When this situation clears up and people are allowed to start coming back to offices, I want to propose being moved to full-time remote work. Does anyone have advice on how to approach this conversation? Should I talk with my boss, HR, or both at the same time? I'm concerned about what happens if they say no, but the company has issued a hiring freeze for the foreseeable future and with my department already short-staffed I could leverage that I will continue to be an asset to them. I don't want to change my plans but I also don't want to be without income.
