How can I work from home every other week?

Hello! I work for a decently sized company (5,000 employees nationwide), and I started a year ago in a big city on the west coast. I moved here with my gf, but she got into a grad school a couple hundred miles north. I see her on most weekends, but I’m going to propose soon and want to live with her.

How can I go about asking my company if I can work from home (in the city that she currently lives) every other week, while still working in the current office every other week? For context, I currently work with a lot of folks on our team in the east coast, so we’re really very video conference and mobile friendly.
