Explaining Poor Transcript Grades due to Mental Health Issues

Hello, I recently graduated undergrad after 5.5 years and have started applying to jobs with some success. One of these opportunities wants me to send them my unofficial transcript as part of the recruitment process.

My problem lies in that for the first 3 years of college I was in a difficult STEM major and due to a variety of factors, in my 3rd year I did not pass almost all of my classes and was placed on academic probation. I got help from a mental health professional at school and was diagnosed with anxiety, symptoms of depression and ADHD and attended regular counseling for the remainder of my time at school. After my 3rd year, I switched majors to a related STEM field and finished out college with a B+ average in my remaining classes.

When I list my GPA on my resume, I use the grades from my general education classes which I did really well in and my 2nd major, which is different from my cumulative undergrad GPA which is quite low due to my 3rd year. Now that I'm submitting my transcript, the difference will be apparent.

How do I explain my situation to jobs that I'm applying to, and will I be rejected due to my transcript and will they perceive me as a liar? Will companies be dissuaded from hiring me due to my past mental health struggles?
