"Can we contact your current manager" screening question sounds sketchy?

I'm applying to a job at a major hospital in New York City and an initial screening question for the online application asked if the company can contact my current manager to discuss my performance. The option answers were:

1) Yes, my manager is aware of my application 2) Yes, but only if I'm a final candidate 3) Yes, but I prefer you to discuss with me first 4)No

My manager is super busy and I don't want her to be troubled too much so I chose #2. Do you think this would auto-screen me out since I didn't put #1? I may be overthinking this...Thanks
