Feeling really unwell and can't get out of bed. I've just completed the first month of my job as well. Is it bad phoning in sick so early?

I'm lying at my bed at 4am wondering what my options are. I've had an extreme back pain and can't get out of bed. I got up to go to toilet and every movement just made me cry in pain. I did used to have back injury before but it's undocumented because I've never went to the doctor's about it before. I've just finished my first month at the job and don't want to look bad phoning in sick. What do I do?
