A few basic questions about Promotions

hey all, have a few questions about promotions and I am soliciting advice from you fine folks. here goes:

1. if your job scope changes dramatically and/or you find yourself doing a variety of tasks with increasing responsibility and visibility, is a promotion warranted or is that just a part of being a good employee?

2. what are your thoughts on tenure vs. performance-based promotions? i.e. your job description or scope changes but because you were promoted a year ago you are not eligible for another promotion (tenure-based)?

3. what are your thoughts on being promoted in title but not compensation. i.e. company reevaluates and determines you are eligible to be bumped in title but only increase compensation 1x/year at year-end?
