Offered a job, what questions should i ask before accepting/declining an offer?

I’ve been offered a position (in the programming field) and it would be my first job that could kick start my career.

They sent me my remuneration package that includes the salary, holidays, insurance etc and it sounds all good.

My biggest concern is time off work. I have a long term health condition and it would require me to take time off work to attend appointments (I get an appointment roughly once every 2 months), what questions should I ask before taking the offer?
