Quitting my job on fmla

I have a health condition that i had to take medical leave for. Turns out i got some bad news and need extended treatment. I opted for giving my two weeks, but my therapist suggested against it as i was out of this corporate job for 4 months. I plan on changing my career from engineering to something on the lines of medicine and go back to school.

Questions i have are:

If i quit, do you expected any negative backlash from my boss? Therapist suggested not telling him now as im using the insurance and benefits i paid for to cover my healthcare expenses

1. Am i making a mistake to chase for my dream career instead of sucking it up and doing the corporate 9-5 in an industry i am not passionate about?

1. Could i utilize ADA to extend my benefits even if i quit?

2. Can i ask my boss to lay me off? Or is it best to quit. Should i give 2 weeks in my situation?
