Application wants phone number for past employer that is now out of business... what do I do?

I am applying for my first nursing job after graduating and the application wants all of my work history for the past ten years. I lived in Florida for a few months 6 years ago and worked some random restaurant job for 3ish months before moving back to my home state. The restaurant is now closed and I can’t even remember my managers name. The app wants the employer phone number and WILL NOT LET ME SUBMIT without it... I tried just putting all zeros but it won’t accept it. I thought about just not including it but I am too scared they’d find out and think I was dishonest for omitting. This is what happens when I try to do the right thing 🤦🏻‍♀️ What do I do?
