Official Application AFTER job offer

So, I'm applying for this position as an Office Administrator for a very successful company. I've never had a position like this before, and I'm very excited! I have been to two interviews so far, and yesterday I received an official job offer!
However, I found out that I need to fill out a more "official" job application before we move forward in the hiring process (drug test, background check, etc.) This is making me nervous, because 1) one of the questions is how many jobs I've had in the last 7 years, and I have to list EVERY single job. Two of the positions I was only at for 1 month. and 2) for a couple of weeks, I was a cocktail waitress at a strip club, and for a few months after that, I was a dancer (I know I know, I've heard all the cocktail waitress/stripper jokes.) I don't particularly want to include that on my job application, but I'm just not sure if that will show up on my background check.

Every where I look says that the company I'm applying for does a criminal background check, but I'm just concerned that if I leave it out on the application, and they DO run a background check and see it, that 1) they'll wonder why I lied and left it out. and 2) they will straight up not hire me because I worked at a strip club. I don't know if my period of time as an "independent contractor" would necessarily show up, but my time as a cocktail waitress may because I received a paycheck and it was taxed.

The positions I've chosen to list on my resume and the qualities I displayed during the interview were enough to impress them, despite not having prior experience as an Office Administrator, and they have seemed very happy with my attitude and professionalism thus far. But I'm very, very nervous about this situation. I really want this position and I have doing a lot of research for it, and I just don't know what I should do.

I've also never had a position have me do an application after extending a job offer, so I'm just not exactly sure what that means. Are they still considering me? Will they rescind the job offer?

If anybody has any advice or insight, I would really appreciate it. I want to fill out and submit the official job application as soon as I possibly can. Thank you!

TL;DR- I'm in the hiring process of a corporate job. I have received the job offer, but have to list all previous employers. Will the time I spent working at a strip club appear on my (criminal) background check?
