How long before a promotion?

I've been in a handful of positions but am relatively still young. In my first job I was seeing raises ~every 6 months. In my last job I didn't see any over 10 months. I just started a new position, not a big fan of my title (mismatch to the amount I actually do) and the pay is a bit low but I love the job and the people I work with. I plan to work my ass off and grow in the position.

When would you bring something like this up with an employer?
