fudging employment history on application after receiving offer?

hey guys,

I graduated in May, and started my first full time job in July. I was terminated in August, for "not meeting expectations." I was only there for a month.

I now have attended an interview and received an offer for a different position that I am more excited about. I did not mention my firing in my interview, and now they're requesting I fill out an application with my list of previous employers. I didn't mind leaving the 1 month job off my resume, but I am afraid leaving off the application may bite me in the ass??

please, any advice is greatly welcome. I'm extremely panicky about this.
